Several changes made, plus

Several changes made, plus some stuff was already there:
- Comments are now oldest-first (easy to do, just took me a while to get to it).
- I also removed the comment ordering selection from the normal display. It seemed distracting to me.
- Made a change so that the comment form is at the bottom of the list.
- Single click subscription has been there. At the bottom of the comments is a "Notify me" checkbox.
- You can set the default status of this checkbox on your User Account page.
